Cagrilintid

A long-acting analogue of the body's own hormone amylin, studied in clinical trials for metabolic and satiety regulation, alone and in combination with semaglutide.

Cagrilintide is a long-acting amylin analogue developed by Novo Nordisk. It is being studied both as a standalone substance and in a fixed combination with the GLP-1 compound semaglutide, marketed under the name CagriSema, in phase 3 trials. To date, cagrilintide is not approved as a standalone medication in any country.

• In a randomized phase 2 trial testing several amounts in 706 adults with overweight or obesity, cagrilintide alone over 26 weeks led to a mean body weight loss of 10.8 percent at the highest amount tested, compared with around 3 percent under placebo. • In a diet-induced obesity mouse model, cagrilintide's body-weight-lowering effect depended on the brain amylin receptors AMY1 and AMY3, and the effect was absent in receptor knockout animals. Carvas AO et al. 2025, eBioMedicine, PMID 40609154
• Structural optimization with fatty-acid lipidation gave cagrilintide a substantially longer half-life of about 159 to 195 hours than the body's own hormone amylin, allowing once-weekly administration in the trial setting. Frequently asked questions

Is cagrilintide already available as a medication?
No. Cagrilintide is being studied as a standalone substance and in the CagriSema combination in phase 3 trials. An FDA approval process for CagriSema has been underway since December 2025. Cagrilintide is currently not an approved medication.

What sets cagrilintide apart from semaglutide?
Semaglutide acts on the GLP-1 receptor, while cagrilintide acts on amylin receptors in the brain. Both compounds are also studied as a fixed combination called CagriSema, which showed a greater effect on body weight in trials than either substance alone.
